    De quervain tenosinovitinde endoskopik yardımlı tek portal birinci Ekstensor kompartman gevşetme

    Amaç Dequervain tenosinovitinde konservatif yöntemler fayda sağlamadığında cerrahi tedaviler uygulanmaktadır. Tekniklerin ilerlemesiyle geleneksel açık cerrahiler yerini endoskopik yöntemlere bırakmaktadır. Amacımız tek portal üzerinden endoskop yardımıyla uygulanan birinci ekstansor kompartman gevşetme uygulanan hastalarımızın sonuçlarını sunmaktır. Gereç ve Yöntem Aynı cerrah tarafından 2019-2021 tarihleri arasında De Quervain tenosinoviti nedeniyle konservatif yöntemlerden fayda görmeyen 8 hastaya endoskopik 13 hastaya ise geleneksel açık yöntemle birinci ekstensor kompartman gevşetme uygulanmıştır. Hastaların demografik özellikleri, ameliyat öncesinde ve sonrası 2. hafta 6. hafta ve 12. haftalarda VAS, qDASH skorları ve kavrama güçleri retrospektif olarak elde edildi. Ek olarak 12. Haftada insizyon skarlarının uzunluğu ölçüldü ve hastalar tarafından kozmetik olarak yara yerleri puanlandırılarak (1 en kötü, 5 en iyi olacak şekilde) veriler kaydedildi. Sonuçlar Çalışmada toplam 17 kadın 4 erkek hasta olmak üzere toplam 21 hasta değerlendirilmiştir. Şikayet başlangıcından ameliyata kadar geçen süre 82 gün olarak saptanmış ve gruplar arasında anlamlı fark saptanmamıştır (p>0,05). Her iki grupta da hastaların ameliyat öncesi VAS, qDASH ve güç ölçümlerinde farklılık gözlenmemiştir. Endoskopik gevşetme uygulanan grupta ameliyat sonrası 2. haftadaki VAS skorlarının anlamlı olarak daha düşük olduğu görülmüş ancak sonraki takiplerde VAS ve qDASH skorları ve kas gücü değerlendirmelerinde fark saptanmamıştır (p=0,001). Skar boyutunun endoskopik grupta anlamlı derecede daha az olduğu ve hasta skar memnuniyetlerinin de yüksek olduğu görülmüştür (p=0,001). Sonuç De Quervain tenosinovitinin cerrahi tedavisinde birinci ekstensor kompartman gevşetmenin endoskop yardımıyla yapılması erken dönemde daha düşük VAS skoru ve kozmetik açıdan daha iyi sonuçlar sağlamaktadır.     Effects of repeated use of disposable FURS on efficiency, safety and cost analysis

    In this study, we aimed to examine cost analysis and evaluate the results of retrograde intrarenal surgery (RIRS) performed with reusable and disposable flexible ureteroscope (FURS) devices in our clinic. In total, we included 96 patients who underwent RIRS surgery for kidney stones with reusable FURS (n = 49) and disposable FURS (n = 47). There were noted preoperative demographic data and stone characteristics of the patients. Also, operation time, fluoroscopy time, hospitalization time, urethral catheter removal time, return to work time, complication rates and cost per procedure were collected and analyzed. There were no significant differences between each groups in terms of demographic characteristics of retrospectively analyzed the data of 96 patients. In addition, the comparison made in terms of operative and postoperative results in both groups was similar. We determined significant difference for mean cost per case compared two groups. There were cost per case $ 293,87 in group 1 and $ 191,48 in group 2. We determined that the repeated use of disposable FURS compared in terms of cost analysis is a safe, effective and low cost method without increasing the infection frequency. © 2022 Ondokuz Mayis Universitesi.     Spinal augmentation: how to choose the best candidate?

    The overall management principles for treating spinal injuries focuses on maximizing clinical outcome by obtaining and maintaining spinal stability and optimizing neurologic function. An optimal treatment method should reduce the detrimental effects of injury, reduce pain and suffering, improve functional outcome and quality of life, provide the best outcome with the least amount of associated morbidity. In osteporotic patients without neurological deficit; we can do indirect reduction and kyphoplasty, indirect reduction and titanium vertebral body stent or percutaneous cement augmented pedicle screw fixation and kyphoplasty or titanium vertebral body stent. In the presence of neurological deficit; anterior decompression with mini open or endoscopic route is necessary.

    Renal hilus ligation with single stapler in laparoscopic donor nephrectomy

    Background. Ligation of renal hilus is the most important stage of laparoscopic donor nephrectomy. Laparoscopic staplers are securely used for renal pedicle control. We present our donor nephrectomy cases in which we used 1 stapler for renal artery and vein ligation. Methods. Demographic data, number of arteries and veins, ligation types, operation time, and complication rates are recorded. Results. One hundred twenty laparoscopic donor nephrectomy cases who were operated between December 2017 and August 2018 in Istinye University Hospital and Istanbul Aydin University Hospital were retrospectively evaluated. All of the operations were done by 2 surgeons with a fully laparoscopic method. None of the cases were converted to open nephrectomy. There was 1 renal artery in 110 (91.7%) cases, 2 renal arteries in 9 (7.5%) cases, and 3 arteries in 1 (0.8%) case. Renal artery and vein were ligated with single stapler in 115 (95.8%) cases. Double stapler was used in 5 (4.2%) patients. There were no major complications for donors and no implantation problems for grafts. Discussion. Laparoscopic donor nephrectomy is the most used technique for living donor operations. Vascular stapler is securely used for renal artery and vein ligation with high costs. Two or, due to the number of vessels, sometimes 3 staplers are used in the standard technique. In our study, the operation was finished securely in 95.8% of the patients with single stapler use. Single stapler use for ligating renal hilus is safe in kidneys even with suitable multiple arteries and veins in laparoscopic donor nephrectomy.

    Renal transplantation in high immunological risk patients: A single-center experience

    Background. Renal transplantation (RT) in high-risk patients is increasingly performed due to an inadequate organ pool and increased rate of RT after a failed transplantation. Safety and prognosis of RT in such patients with high risk is an ongoing debate. Herein we aimed to present our single-center experience on RT of high-risk patients. Methods. A total of 89 consecutive RT patients were included into this study in a 10-month period. Patients were divided into 3 groups: the low-risk group (n = 47) with negative panel reactive antibody (PRA), medium-risk group (n = 18) with positive PRA but mean fluorescence intensity (MFI) <2000, and high-risk group (n = 24) with positive PRA and MFI >2000 or donor specific antibody (DSA) positivity. Groups were compared in terms of demographic features, serum creatinine levels, acute rejection rates, delayed graft function (DGF), and patient or graft loss. Results. Age of the recipients were similar between the groups. Desensitization (7% vs 11% vs 42%, respectively, in low-, medium-, and high-risk groups; P = .001), plasmapheresis (6% vs 11% vs 46%, respectively, P < .001), and rituximab treatments (0% vs 0% vs 25%, respectively, P < .001) were significantly more frequently performed in high-risk patients. Serum creatinine levels at 1 month and 6 months after RT were similar between the groups (P = .43 and P = .71, respectively). Rates of acute rejection (6% vs 6% vs 16%, respectively, P = .52) and DGF (9% vs 11% vs 29%, respectively, P = .15) were similar between the groups. Frequencies of loss of patient or graft were also similar (0% vs 6% vs 4%, P = .15). Conclusion. RT may be successfully performed in high-risk patients without an increase in the risk of acute rejection, DGF, or patient/graft loss.

    Comparison of quality of life between early and advanced stage ovarian cancer patients

    Approximately 225,000 new cases of ovarian cancer are diagnosed worldwide each year. The standard approach for treating patients with ovarian cancer is surgery followed by platinum and taxane based chemotherapy (CT). Quality of life (QoL) and well-being are affected by different physical, social, spiritual and mental factors. Immediately after diagnosis of cancer, most patients focus on anticancer treatment and its challenges. In this situation the primary objective of treatment is to cure the disease and secondary to maintain the best possible QoL. Since survival rates and life expectancy after treatment for cancer are rising due to more effective therapies, so more focused on QoL.
