    Gut microbiota-derived metabolite trimethylamine N-oxide and biomarkers of inflammation are linked to endothelial and coronary microvascular function in patients with inflammatory bowel disease

    Background: Inflammatory bowel disease (IBD), which is an umbrella term used for ulcerative colitis (UC) and Crohn's disease (CD), is associated with an increased risk for atherosclerotic cardiovascular disease (CVD). We aimed to investigate the association of local and systemic biomarkers of inflammation and gut microbiota-derived metabolite trimethylamine N-oxide (TMAO) with endothelial and coronary microvascular dysfunction in IBD. Methods: A total of 56 patients with IBD (20 with UC and 36 with CD) and 34 age and gender matched controls were included. For all participants, samples were collected to analyze faecal calprotectin, and TMAO concentrations. Ultrasound-based examinations were done to measure flow-mediated vasodilatation (FMD) and coronary flow velocity reserve (CFVR). Results: Patients with IBD had lower CFVR (2.07 (1.82–2.40)) and FMD (8.7 ± 3.7) as compared to controls (2.30 (2.07–2.74), p = 0.005 and 11.9 ± 6.8, p = 0.03). In patients with IBD, TMAO concentration (r = ?0.30, p = 0.03), C-reactive protein (r = ?0.29, p = 0.03) and WBC count (r = ?0.37, p = 0.006) had a significant negative correlation with CFVR, and TMAO (? = ?0.27, 95 % CI: ?0.23 to ?0.02) and WBC count (? = ?0.31, 95 % CI: ?0.56 to ?0.06) were significant predictors of CFVR after multivariate adjustment. None of the biomarkers of inflammation or TMAO showed significant correlations with FMD. In patients with UC, TMAO showed a significant correlation with both CFVR (r = ?0.55, p = 0.01) and FMD (r = ?0.60, p = 0.005) while only WBC count had a statistically significant correlation with CFVR (r = ?0.49, p = 0.004) in patients with CD. Conclusions: Gut microbiota-derived metabolite TMAO and biomarkers of systemic inflammation are associated with measures of endothelial/coronary microvascular dysfunction in patients with IBD. © 2022

    A review on hydrogen sulfide: Is it pro-nociceptive or anti-nociceptive?

    Pain is sensed by the activation of painful nociceptors in the periphery or by pain mediators, such as bradykinin, serotonin, histamine, and prostaglandin, released from the damaged tissue, afferent transmission to the medulla spinalis, and by transmission stages to the high centers over the dorsal horn. Pain, which was seen as only a warning sign in the past, is now accepted as a phenomenon in itself that needs to be treated and the search for new, stronger active substances with fewer side effects in the treatment of pain is in demand. Hydrogen sulfide (H2S) is a modulator of T-type Ca+2 channels, especially in Cav3.2, which are known to play a critical role in the processing of pain. H2S can also show an anti-nociceptive effect by opening K+ channels and blocking nociceptors. Exciting preclinical data has demonstrated that H2S-derived Non-steroidal anti-inflammatory drugs (NSAIDs) and analgesic agents can be used to treat various types of pain. H2S increases the resistance of gastric mucosa against injury occurred by drugs used for pain relief and accelerates its repair, so it provides an advantage to derivatized drugs. In addition, H2S donors have also been shown to induce analgesia through mu-opioid receptors. Based on the studies, it is thought that the combination of H2S with opioid receptor agonists may provide an additive or even synergistic analgesic effect. It is estimated that the modification of H2S, with currently used analgesic drugs to prevent various side effects and increase analgesic effects, is a promising and wise approach.

Our aim with this presentation was to share our center’s experience on this subject. Methods: Hematopoietic stem cell transplantation was performed in a total of 69 patients with a diagnosis of high-risk acute lymphoblastic leukemia between 2015 and 2020 at our center. The demographic and treatment-related information of the patients was retrospectively evaluated.

    Demographic, clinical, and laboratory characteristics of elderly onset psoriatic arthritis

    Background: Psoriatic arthritis (PsA) is a chronic in?amatory disease characterized with axial and peripheral joints involvement. It is rarely affects patients older than 65 years old. Objectives: The purpose of this study is to compare and evaluate the demographic,clinicalandlaboratoryfeaturesofelderly-onsetpsoriaticarthritis(EOPsA) and young-onset (YOPsA) patients. Methods: One hundred and eighty patients diagnosed with PsA according to CASPAR criteria and followed-up in single center were included in this study. The patients with initial symptoms started after age 65 were accepted as EOPsA. Demographic,clinical,andlaboratorydataandthemedicationswhichthepatients recieved were recorded and retrospectively evaluated. Results: Nineteen (10.5%)of 180 patients were diagnosed as EOPsA, and 161 (89.5%) patients were evaluated as YOPsA. Mean patient age was 42.1years for YOPsA group and 68.3years for elderly onset group. Mean duration of disease was 5.6 years for early onset group and 1.3 years for elderly onset group (p=0.001). Fourteen (73.3%) of 19 EOPsA patients were female and 5 of them were male. Higher rates of fatique, pain scores, comorbid diseases and acute phase reactants were detected in EOPsA patients comparing to YOPsA (p=0.000, p=0.000, p=0.001 and p=0.001 respectively). YOPsA patients have more dactilitis, nail involvement, elevated PASI scores, and smoking habitus when comparedwithEOPsApatients(p=0.019,p=0.03,p=0.005,p=0.004respectively). In terms of the treatment options chosen, there was no signi?cant difference in the use of non-steroidal anti-in?ammatory drugs (NSAIDs), corticosteroids (CS), methotrexate (MTX), and sulfasalazine (SSL), but there was a more frequent use of anti-tumor necrosis factor -alpha in YOPsA group. Conclusions: Herein we showed that YOPsA and EOPsA patients may have different demographic, clinical, and laboratory features. EOPsA patients are characterized with higher rates of fatique, pain scores, comorbid diseases, and acute phase reactants and less dactilitis, nail involvement and anti-TNF-alpha usage.

    Development of independent mu/treatment time verification algorithm for non-imrt treatment planning: a clinical experience

    The aim of this study is to develop an algorithm for independent MU/treatment time (TT) verification for non-IMRT treatment plans, as a part of QA program to ensure treatment delivery accuracy. Two radiotherapy delivery units and their treatment planning systems (TPS) were commissioned in Liv Hospital Radiation Medicine Center, Tbilisi, Georgia. Beam data were collected according to vendors' collection guidelines, and AAPM reports recommendations, and processed by Microsoft Excel during in-house algorithm development. The algorithm is designed and optimized for calculating SSD and SAD treatment plans, based on AAPM TG114 dose calculation recommendations, coded and embedded in MS Excel spreadsheet, as a preliminary verification algorithm (VA). Treatment verification plans were created by TPSs based on IAEA TRS 430 recommendations, also calculated by VA, and point measurements were collected by solid water phantom, and compared. Study showed that, in-house VA can be used for non-IMRT plans MU/TT verifications.
