Öğe Examining the relationship between violence against healthcare workers and their levels of exhaustion, confidence and safety(Ege University, 2024) Celbiş, Bahar; Özaydın, ÖzlemBackground: This study aims to determine the relationship between violence against healthcare workers and their burnout, confidence, and safety levels. Methods: The descriptive and cross-sectional study sample consisted of 535 healthcare workers working in a foundation hospital. Data were collected through a questionnaire. The questionnaire included demographic questions, research questions about violence, "Safety and Confidence Scale of Healthcare Professionals" and "Maslach Burnout Scale". Student's t, Mann Whitney-u, Kruskal-Wallis, Chi-Square, Pearson Correlation, and Linear Regression tests were applied. Statistical significance was accepted as 0.05. Findings: The majority of the participants were female, between the ages of 26-35, single, and had an associate degree. The perceived safety level of those exposed to violence decreases (p<0.001), and burnout level increases (p<0.001). Injury as a result of violence significantly decreases the perceived safety level (p=0.019). The status of being subjected to violence does not vary according to the employees' demographic characteristics or working life data. Conclusions: Violence decreases employees' perceived safety level and increases burnout. It is recommended to implement psychological support or rehabilitation programs for healthcare workers to reduce violence-related burnout and increase confidence and perception of safety.Öğe A worldwide bibliometric analysis on euthanasia(SERNEV Ar-Ge Yazılım Yayıncılık Eğitim Danışmanlık ve Organizasyon Ticaret Limited Şirketi, 18.11.2024) Güdük, ÖzdenIntroduction: Euthanasia, which allows individuals with an incurable disease to end their lives in an honourable way, is a long-debated practice. It is related to various issues such as culture, philosophy, religion, ethics, public value, and efficient use of health resources. While some advocate for the right of individuals to choose euthanasia, others criticize it on ethical, legal, and religious grounds. Methods: In this study bibliometric analysis was used and the data were obtained from the Web of Science Core Collection database on July 26, 2024. Only English publications and articles were included using keywords such as "euthanasia," "end-of-life decisions," "medical assistance in dying," "physician-assisted death," and "physician-assisted suicide" without time restrictions. VOSviewer software was used to create network and density graphs, and SciMAT software was used to create strategic diagrams. Results: A total of 2,230 studies published between 1980 and 2024 were analyzed. The author with the most publications is Deliens, L, with 86 studies. The majority of studies were published in the USA, Netherlands, and Canada (692, 336, 225 respectively). The most frequently used keywords are euthanasia, palliative care, and ethics. The highest number of publications (149) was in the year 2021. In the strategic diagram for the first period (between 1980 and 2001), the themes of care and ethics were prominent, while for the second period (between 2002 and 2024), the clusters of nurse and shared decision-making were notable. Conclusion: After its legalization in the Netherlands, euthanasia has been accepted and implemented in some countries over the years, with variations in scope. Some countries, like the Netherlands and Belgium, also accept health problems related to old age and mental illness as euthanasia requests. The discussion around euthanasia often revolves around the principles of medical ethics, including autonomy, beneficence, non-maleficence, and justice. Studies have focused on the role of physicians and nurses in the process, as well as end-of-life decision-making by patients. Aim: It was aimed to compare two tools used to evaluate fall risk in hemodialysis patients. Method: The mean age of the patients was 58.72 ± 14.49 years and 62% were male. The mean duration of treatment was 4.98 ± 4.71 years. Within the last year, 33% of the patients had a history of falling. The patients' fall risk was assessed using the Itaki Fall Risk Scale and Dialysis Fall Risk Index. The relationship between patients' age, gender, duration of hemodialysis treatment, and the status of having a chronic disease and falls were examined. Results: According to the Itaki Scale, 57% were in the ''high-risk'' group. According to the Dialysis Fall Risk Index, 64% were in the "very high risk" group. The mean Itaki Fall Risk Scale score was 4.75 ± 3.92, and the mean of the Dialysis Fall Risk Index was 7.59 ± 1.92. According to the cut-off score, the sensitivity and specificity of the Itaki Fall Risk Scale were 97% and 99.6%, and the Dialysis Fall Risk Index was 93.9% and 99.6%. The positive and negative predictive value were 56.1% and 97.7%, respectively for the Itaki Fall Risk Scale. These points were 51.6% and 100% for the Dialysis Fall Risk Index. Conclusion: Both scales were effective in improving care quality in hemodialysis centers. Yapay zeka teknolojilerinin insan düşüncesinden daha yüksek performans gösterme olasılığı tüm mesleklerde olduğu gibi hemşirelik mesleğinde de tehdit olarak görülebilmektedir. Hemşireler profesyonel sağlık ekibinin vazgeçilmez bir parçasıdır, mesleki gelişim açısından teknolojiyi anlama ve aktif kullanımları mesleğin geleceği açısından önemlidir. Teknolojik gelişmelerin hemşirelik mesleğine ciddi etkileri olacağı açıktır. Hemşireler rutin ve tekrarlayan işlerini robot teknolojisine devredebilecekler. Bu durum robot hemşireler, hemşirelerin yerini alabilir mi? Hemşirelik mesleği son bulur mu? gibi soruları düşündürmektedir. Hemşireler yapay zeka ve teknolojiye uyum sağladıklarında robot hemşirelerin kullanım kolaylığı ve avantajlarından yararlanacak, hasta ile duygusal bağ kurması gereken bakım gibi karmaşık işlere vakit ayırabilecektir. Robot hemşireler henüz yetkin olmadığı için, insiyatif kullanma ve eleştirel düşünme gibi süreçlerde hemşirelerin yeri daha önemli olacaktır. Çalışmada Covid-19 Pandemisinin bilinen ya da henüz bilinmeyen etkilerinin elemine edilmesi amacıyla 2018 yılı verilerinin kullanımı daha uygun görülmüştür. Ayrıca, İstanbul ili, yoğun popülasyon sebebiyle istisnai durumu düşünülerek çalışmaya dahil edilmemiştir. Araştırmada yöntem olarak, illerin hastalık bazında ölümlerinin toplam ölümlere oranı ve tüm il nüfusuna oranı hesaplanmıştır, Bulgular küçük değerden başlayarak sıralanmış ve en küçük değere sahip yani performansı en yüksek 15 il verileri değerlendirme için kullanılmıştır.Öğe Investigation of the Knowledge Level of Hospital Managers About Strategic Management Tools and Their Use(Dilaver TENGİLİMOĞLU, 27 Kasım 2024) Güzel, Tülay; Özaydın, ÖzlemObjectives: Effective and efficient implementation of all elements of strategic management is important for health organizations to achieve their future goals. This study aims to examine the knowledge levels of hospital managers about strategic management tools, their usage of these tools, and the priorities and needs in tool selection. Materials and Methods: The population of the study is the managers working in private hospitals in Antalya Province. 90 managers constitute the sample of the study. The data were collected by face-to-face questionnaire method. The questionnaire includes sociodemographic characteristics form and statements to measure the level of knowledge and use of strategic management tools. Mann Whitney U and Kruskal-Wallis tests were applied to examine the differences between groups, and Spearman Correlation Analysis was applied to examine the relationships between variables. SPSS.25 Package Program was used to analyze the data obtained. Statistical significance was accepted at 0.05 level. Results: 59% of the participants were female, 63% had a bachelor's degree, and the majority (41%) were middle managers. Those who did not receive managerial training and strategic management training were the majority (54% and 71%, respectively). When the knowledge level of the managers about strategic management tools was analyzed, mission and vision (98.9%) received the highest score, and value chain analysis (73.3%) received the lowest score. It was observed that the priorities of the administrators in the selection of strategic management tools were different between the groups according to gender, level of education, and type of school graduated from. In addition, statistically significant relationships were found between the priorities in the selection of strategic management tools and age, professional experience and years of working in the organization. Conclusion: Managers have limited knowledge and use of strategic management tools and there are differences in the selection of strategic management tools according to their sociodemographic status.Öğe Investigating The Effect of Perceived Empowerment on Artificial Intelligence Anxiety Levels in Healthcare Workers(Birleşik Metal İş Sendikası, 22 Ocak 2025) Güdük, Özden; Vural, Ayten; Dişiaçık, GülerThe aim of this study is to explore the correlation between AI anxiety and the perceived empowerment of healthcare professionals. An online survey was conducted among healthcare professionals at a training and research hospital. The survey included questions about the participants' socio-demographic characteristics, as well as the AI Anxiety Scale and the Perceived Empowerment Scale. A total of 285 healthcare professionals completed the survey between December 2023 and February 2024. Healthcare professionals AI anxiety at a level slightly above the medium, while their perception of empowerment is high. The level of AI anxiety varied based on factors such as gender, age, total years of work, and the specific unit they work in. Similarly, the perception of empowerment differed among groups based on age, total years of work, and marital status. The study also found a negative relationship between the meaning-competence dimension of perceived empowerment and the AI learning dimension, as well as a positive relationship between the AI sociotechnical blindness dimension. It was found that individuals with a high perception of empowerment are less anxious about learning new information about artificial intelligence, but more anxious about the potentially harmful and dangerous aspects of artificial intelligence. The study suggests that empowerment, as an effective human resource management tool, can be utilized by health managers to alleviate employees' AI nxiety.Öğe Investıgatıon Of Turkısh Nurses' Attıtudes Towards Braın Draın(Haşim Çapar, 16 Ocak 2024) Özaydın, Özlem; Vural, Ayten; Güdük, ÖzdenBackground and Aims: In recent years, there has been a significant brain drain in the health sector due to healthcare workers migrating abroad to work. This study analyzes the push and pull factors that prompt nurses to leave the country. Methods: The sample of the cross-sectional and descriptive study consists of 270 nurses working in Istanbul. The researchers used a questionnaire to collect data on the nurses' demographic information, work history, and attitudes toward brain drain. The data collected was analyzed using statistical tests such as Chi-Square, Independent two sample T-test, Kruskal Wallis, and Pearson Correlation test. A statistical significance level of 0.05 was used to determine the significance of the results. Results: The mean age of the nurses was 33.6±8.95 years, and the mean working period was 10.8±9.27 years. Most were female, married, and had a bachelor's degree. Fifteen percent of the participants had attempted to go abroad, 53% were only at the thought stage, and 32% stated that they did not want to go abroad. The scores for both push and pull factors were significantly higher among those who wanted to go abroad than those who did not (p<0.01). Additionally, nurses working in specialized units had more positive thoughts about migration abroad (p<0.05). A negative correlation was found between age and the pull factor score (p<0.05), and working time in the profession was negatively correlated with push and pull factor scores (p<0.01). However, there was no difference in the mean scores of the push and pull factors among those who stated that they wanted to go abroad regarding gender, marital status, length of service, and units of employment. Conclusions and Suggestions: Young nurses, nurses with fewer working years, and nurses working in specialized units have more positive attitudes toward brain drain. Nurses are an integral part of the healthcare workforce, and Türkiye may face severe consequences due to the potential effects of brain drain in the near future. Therefore, policymakers in the healthcare sector must take necessary measures to prevent this from happening.Öğe Investigation of Fires Occuring in Hospitals Between 2019-2023 in Türkiye: A Content Analysis(Ankara Üniversitesi, 30 Kasım 2024) Güdük, ÖzdenFires are one of the most important dangers for hospital buildings. The aim of the study is to examine the hospital fires occurred between 2019-2023 in Türkiye. In the study, fires in public and private hospitals operating in Türkiye between 2019 and 2023 were examined. In this cross-sectional study, news about fires in hospitals in Türkiye were obtained from the archives of 7 national and 3 local news agencies using the search terms "fire" and "hospital" in Turkish. The content analysis was used to evaluate news texts. Out of a total of 76 hospital fires, 3 resulted in deaths caused by fire, leading to a total of 16 patient fatalities. Additionally, in all fires, a total of 99 individuals, including both patients and employees, required medical treatment due to the fire. In 19 of the fires, patients had to be evacuated due to exposure to either flames or smoke. The highest number of hospital fires occured in İstanbul. While the cause of 28 fires remains unknown, 27 fires were caused by electricity. Hospitals should prioritize fire prevention measures due to the potential losses and difficulties in evacuation. Regular checks of the hospital's electrical installations and devices should be carried out, as well as capacity assessments for new devices and expansions of the building. It is also recommended to keep emergency and disaster plans and teams updated for possible fire situations, increase employee knowledge, and conduct drills.Öğe The Effect Of Vıolence Agaınst 112 Emergency Healthcare Personnel On Job Satısfactıon(Rojan Gümüş, 29 Aralık 2023) Dinçler, Serpil; Hölümen, Nazlı; Yükseloğlu, Emel Hülya; Erkan, ItırIn this study, the types of violence faced by the healthcare personnel working in the 112 Emergency Health Services and the effect of this situation on the job satisfaction of the healthcare personnel were examined. Questionnaire was applied to 165 emergency healthcare personnel working in Istanbul, between August and October 2021. Demographic information and Hackman and Oldham job satisfaction scale were used in the questionnaire. 80.6% of the participants stated that they were exposed to physical, verbal, psychological or sexual violence. It was determined that 48.7% of the perpetrators were the relatives of the patient and 30.2% were the patients themselves. In addition, it was determined that 78.9% of the attackers were male Significant differences were found between the duration and age of healthcare workers in the institution and their job satisfaction. It has been determined that the job satisfaction of healthcare personnels who are exposed to violence is lower than those who are not exposed to violence. In the state of exposure to violence, the motivation and psychological state of the personnel working in the emergency health services are highly affected, and this has a negative effect on job satisfaction. It is recommended that health managers take measures to protect the health and safety of healthcare personnels and take initiatives to increase job satisfaction.Öğe Covid-19 pandeminin sağlık çalışanlarında vicdani zekânın vicdan algısı üzerindeki etkisi(TR- Dizin, 2021) Arslanoğlu, Ali; Tütüncü, Döne; Günay, Aylin; Bektemur, GüvenVicdan sağlık çalışanlarını yanlış yapmaya karşı uyaran, rehberlik eden önemli bir kavramdır. Bu bağlamda sağlık çalışanlarında vicdanın araştırılması önemlidir. Çalışmanın amacı, sağlık çalışanlarında vicdani zekânın vicdan algısı üzerindeki etkisini değerlendirmektir. Araştırmanın örneklemini Türkiye’de görev yapan 389 sağlık çalışanı oluşturmaktadır. Araştırmada daha önceden geçerlilik ve güvenilirliği yapılan ölçeklerden oluşan anket kullanılmıştır. Ölçeklerin güvenirliği Cronbach Alpha yöntemi ile test edilmiştir. Verilerin analizinde SPSS Programı kullanılmıştır. Verilerin analizinde Pearson korelasyon, ANOVA Testi, t-Testi ve Regresyon Analizi kullanılmıştır. Araştırmada vicdani zekâ ve vicdan algısı ortalamalarına göre cinsiyet ve eğitim durumu açısından anlamlı bir farklılık bulunmamıştır. Öte yandan medeni durum ve meslek grupları açısından anlamlı bir farklılık gözlemlenmiştir. Yine vicdani zekâ ve vicdan algısı ortalamalarına göre yaş önemli bir belirleyici olarak belirlenmiş olup, literatürde farklı çalışmalar bulunmaktadır. Çalışmada vicdani zekâ ile vicdan algısı arasında pozitif yönlü, orta düzey bir ilişki tespit edilmiştir (r=,527, p<,001). Ayrıca vicdani zekânın vicdan algısı üzerine etkisi istatistiksel olarak anlamlı (p<,001) olduğu ve vicdani zekânın vicdan algısının %28’sini açıkladığı bulunmuştur. Çalışmada sağlık çalışanlarında vicdani zekâ düzeyinin vicdan algısını olumlu yönde etkilediği sonucuna ulaşılmıştır.Öğe COVID-19 salgını döneminde sağlık çalışanlarının iş doyumu(DergiPark, 2022) Özaydın, Özlem; Vural, Ayten; Güdük, ÖzlemIn this study, it was aimed to reveal the effects of the COVID-19 epidemic on the job satisfaction of healthcare professionals working in a hospital. An online questionnaire containing questions on the socio-demographic characteristics and their working status in the COVID-19 outbreak and the Minnesota Job Satisfaction Scale was applied to healthcare professionals working in a training and research hospital. 268 people volunteered to participate in the study answered the questionnaire between June and August in 2020. The job satisfaction of those who were single, diagnosed with COVID-19, and who had a quarantine process were found lower. The job satisfaction of those who are not actively involved in patient care were higher than others. It was found that the job satisfaction of those working in clinics where COVID19 patients are treated, those staying in temporary accommodation centers during the pandemic, and healthcare professionals with weekly working time of 45 hours or more were lower. It is needed to take measures and encourage healthcare professionals - especially those working in units where COVID-19 patients are treated - to increase their job satisfaction during pandemic. During the pandemic, it is recommended to reduce weekly working hours, support employees who got infected and who had quarantined, and provide healthcare professionals with accommodation under suitable conditions.Öğe Evaluation of the effects of value-based purchasing applications on cost and efficiency(2020) Işık, Mustafa; Gümüş, Emel; Kasap, Ece UysalThe importance and effectiveness of value-based procurement come to the forefront in terms of taking the studies based on value-based cost approach as the basis of the use of medicinal products and drugs, which constitute an important cost item in hospitals, and in particular to understand that the only factor in procurement is not the price. The main purpose of this study is to show the applicability of cost-effectiveness and efficiency studies in medical supply chain management in hospitals by carrying out studies on value-based procurement applications within the scope of value-based health services. The “non-price element” article is included in the Public Procurement Law, the main obstacle to value-based price implementation is the basis of the ceiling price procedure in the SUT (Health Implementation Communiqué). Calculating the actual cost and making decisions based on this account should be of priority. In order to avoid unpredictable cost risk, rational purchasing techniques will need to be applied. When we think that reimbursement systems have recently focused on materials and drug use and how they can reduce these costs, it is a fact that value-based payments will come to the fore in the coming period and many products will not be paid. In this context, when evaluating value-based health services, it is necessary to evaluate the issue from a value-based perspective in all processes of the procurement and medical supply management chain and to ensure that decisions are made taking into consideration the side costs that may occur. In the value-based purchasing approach, patient benefit, efficiency, and innovative initiatives should be at the forefront. The correct realization of the results of these perspectives will be possible with the acquisition of a sustainable value-based purchasing approach. With this approach, medical supply chain processes should be considered as strategic basic processes, not as operational support processes.Öğe Productivity loss and associated costs among patients with transfusion-dependent thalassemia in Dubai, United Arab Emirates(Dove Medical Press, 2021) Alshamsi, Shaikha; Hamidi, Samer; Özgen Narcı, HacerObjective: The aim of this study was to assess the indirect burden of patients with transfusion-dependent thalassemia (TDT) in Dubai, the United Arab Emirates (UAE). Methods: Adult patients with TDT were enrolled during blood transfusion sessions. Productivity loss for patients in the preceding four weeks was reported using the validated iMTA Productivity Cost Questionnaire (iPCQ). Results: A total of 161 adult patients with TDT were included in the study. The annual total productivity loss cost at paid work among 79 employed patients was AED 17,230 (USD 4691) (95% CI: 12,302-22,159), out of which absenteeism contributed AED 10,328 (USD 2812) and presenteeism contributed AED 6902 (USD 1879), whereas the annual total productivity loss cost at unpaid work was AED 4053 (USD 1104) (95% CI: 3471-4661). The annual mean total indirect cost for paid and unpaid work was AED 21,283 (USD 5795). Monthly income, nationality, and having two and more disease complications were the significant predictors of higher productivity loss costs. Having previously had a splenectomy was a negative and significant predictor of productivity loss costs. Conclusion: TDT was associated with substantial productivity loss and indirect costs in the UAE. Health policies that aim to control transfusion-dependent thalassemia effectively are necessary and could result in cost savings for patients and employers.Öğe Gender mainstreaming in health(DergiPark, 2020) Işık, Fikriye; Kıyak, Mithat; Işık, MustafaThe study aims to evaluate gender inequality faced by women working at healthcare services sector and to create awareness. The term “gender” expresses the socially determined roles and responsibilities of women and men and it may vary among populations and over time. Gender equality can be ensured not only through access to healthcare services, professional equality, justice and equity, but also by equal distribution of responsibilities and income between men and women. The study reviewed the literature particularly by screening the recently printed papers on gender mainstreaming, especially in association with the health sector. According to World Economic Forum Global Gender Gap 2020, Turkey is ranked 130 among 153 countries in terms of gender equality. This fact may be secondary to low engagement of women in the labor force. Although working hours of men and women are equal in Turkish health sector, the monthly wage equals to 201.9 hours for men and 200.5 hours for women. Another indicator of gender gap is the gender-based violence against women. According to a study conducted by Ministry of Health in 2018, women are mostly exposed to violence in health sector by 62.5%; 48.1% of healthcare professionals are exposed to verbal violence and 64.9% of crimes of violence are committed by men. Women account for majority of professionals employed in health sector. Considering their health professions, approximately 70% of nurses are women, while the figure is 100% for midwives and approximately 50% for medical doctors. In conclusion, gender-based discrimination may occur in terms of taking advantage of the opportunities, allocation and use of resources and access to services. Women are far worse affected by aforementioned discrimination, as they are more disadvantageous and have lower social status than men. This study advises a perspective that focuses on “gender parity” better regarding policies, strategies and processes in the delivery of healthcare services.Öğe Salgın hastalıkların yönetiminde Nesnelerin İnterneti (NİT) kullanımı: COVID-19 örneği(DergiPark, 2021) Tütüncü, Döne; Esen, Muhammet FevziGiriş ve Amaç: Çalışmada, COVID-19 salgın yönetiminde nesnelerin internetinden (NİT) hangi alanlarda faydalanıldığının nitel araştırma yöntemlerinden biri olan bilim haritalarıyla tespit edilmesi amaçlanmıştır. Gereç ve Yöntem: Sciencedirect ve Pubmed platformları kullanılarak, Ocak 2020–Ocak 2021 arasında COVID-19 salgınının yönetiminde NİT uygulamalarına ilişkin 150 makale incelenmiştir. Bilimsel araştırmaların içerik ve ağ yapılarının tespitinde VOSviewer paket programı kullanılmıştır. Bulgular: NİT’in büyük veri akışının sağlanmasına yönelik ağ optimizasyonu, salgın yayılımının gerçek zamanlı izlenmesi ve tahminlenmesi, sosyal mesafenin korunması amacıyla uyarı sistemlerinin oluşturulması, hastalığa ilişkin biyolojik verilerin ve tıbbi görüntülerin sınıflandırılması, kronik hastaların tespit edilmesi, hastaları ve toplumu yönlendirici mesajlaşma sistemlerinin kullanımı, sağlık iş gücünün planlanması, tele sağlık hizmetleri, medikal NİT, hasta verilerinin güvenliği ve gizliliği, izlem için insansız hava teknolojileri ve salgın yönetiminde maliyet etkinliğinin sağlanması çalışmalarında yoğunluklu olarak gerçekleştirildiği tespit edilmiştir. Sonuç: Çalışma sonucunda, NİT’in salgın yönetiminde önemli bir role sahip olduğu görülmekte olup, NİT’in sosyal mesafenin korunması ve koruyucu önlemlerin alınması, kişisel ve ticari faaliyetlerin sürdürülmesi ve sağlık hizmetlerinin yürütülmesi çalışmalarında kullanıldığı anlaşılmaktadır. Yaşlı ve kronik hasta grubunun sağlık durumları ve fiziksel aktivitelerinin takip edilmesinde giyilebilir teknolojilere odaklanan çalışmaların sayısının arttırılmasının önemli olduğu düşünülmektedir.
