Ameliyat öncesi ağrı korkusunun ameliyat sonrası ağrı ve genel konfor düzeyine etkisi

Özet
Bu çalışma cerrahi hastalarında ameliyat öncesi ağrı korkusunun ameliyat sonrası ağrı ve genel konfor düzeyine etkisinin belirlenerek ağrı korkusuna yönelik hemşirelik bakımının geliştirilmesi amacı ile tanımlayıcı ve kesitsel türde gerçekleştirildi. Çalışmanın örneklemini, bir üniversite hastanesinin genel cerrahi kliniğinde Aralık 2019 ve Şubat 2020 tarihleri arasında cerrahi girişim uygulanan, çalışmaya alınma kriterlerini karşılayan, istekli ve onamı alınmış 203 hasta örneklemi oluşturdu. Verilerin toplanmasında hastaların Tanıtıcı Özelliklerini İçeren Bilgi Formu, Görsel Kıyaslama Skalası, Genel Konfor ve Ağrı Korkusu ölçekleri kullanıldı. Verilerin istatiksel değerlendirilmesinde yüzde, ortalama student t-testi ve ANOVA, Mann Whitney u-testi ile Kruskall Wallis testleri kullanıldı. Çalışmaya alınan hastaların %62,6'sının kadın, % 36'sının 50-65 yaş grubunda, % 48,3'ünün ortaokul mezunu, % 39,4' üne anüs ve rektum cerrahi girişimi uygulandığı belirlendi. Hastaların ameliyat öncesi dönemde orta düzeyde (80,15±22,69) ağrı korkuları, ameliyat sonrası dönemde orta düzeyde (5,17±2,72) ağrı deneyimlediği ve konfor düzeylerinin de genel ve alt boyutlarının iyi düzeye yakın (128,73±12,03) saptandı. Çalışmada kadınlarda ve analjezik kullanan hastaların ameliyat öncesi ağrı korkuları yüksek, evlilerin ağrıyı daha fazla hissettiği, 18-33 yaş grubunda yer alanların konfor düzeyleri düşük (p <0.05) bulundu. Hafif ve orta şiddetli ağrısı olan hastaların ağrı korkularında artma olmasına karşın konfor düzeylerinde azalma olduğu belirlendi (r=0,248; p=0.006). Çalışmada ameliyat öncesi ağrı korkusunun hastaların konfor düzeylerini etkilediği sonucuna ulaşıldı. Ameliyat öncesi hastaların ağrı korkusunun azalmasına/önlenmesine yönelik eğitim, tamamlayıcı ve alternatif tedavi yaklaşımlarından kullanılmasının yararlı olacağı önerilmektedir.
This study was conducted in a descriptive and cross-sectional manner with the aim of improving nursing care for fear of pain by determining the effect of preoperative pain fear on postoperative pain and general comfort level in surgical patients. The sample of the study consisted of 203 patients who had undergone surgical intervention between December 2019 and February 2020 in the general surgery clinic of a university hospital, who met the inclusion criteria, and whose consent was obtained. In the data collection, the information form containing the descriptive characteristics of the patients, the Visual Comparison Scale, General Comfort and Pain Fear scales were used. Percentage, mean student t-test and ANOVA, Mann Whitney u-test and Kruskall Wallis tests were used for the statistical evaluation of the data. It was determined that 62.6% of the patients included in the study were women, 36% were in the 50-65 age group, 48.3% were secondary school graduates, and 39.4% had anus and rectum surgery. The patients experienced moderate (80.15±22.69) pain fear in the preoperative period, moderate (5.17±2.72) pain in the postoperative period, and their comfort levels were close to a good level (128.73±12.03) were detected. In the study, it was found that preoperative pain fear was high in women and patients using analgesics, married women felt more pain, and those in the 18-33 age group had low comfort levels (p <0.05). Although there was an increase in fear of pain in patients with mild and moderate pain, it was found that their comfort levels decreased (r =0.248; p=0.006). In the study, it was concluded that the preoperative fear of pain affected the comfort levels of the patients. It is suggested that it would be beneficial to use training, complementary and alternative treatment approaches to reduce/prevent pain fear of patients before surgery
